Marketplace Order Form This Marketplace Order Form (“Order Form”) is made effective as of the date of signature (“Effective Date”), by and between EatStreet, Inc., a Delaware corporation with offices located at 811 East Washington Avenue, Suite 400, Madison, WI 53703 (“EatStreet”), and Merchant (identified in the following table). EatStreet and Merchant may be referred to herein individually as a “Party” and collectively, the “Parties”.
